The gradual deposition of a hyperphosphorylated tau protein within select neuronal types in specific nuclei or areas is central to the disease process. The staging of AD-related neurofibrillary pathology staging scheme such as the Braak staging to predict the spread of tau pathology as the disease progresses. Thus, the overall aim of this study was to establish and validate a connectivity-based prediction model of patient-specific tau spreading. To address this, we included 18F-flortaucipir tau-PET (i.e., AV1451) Reference Braak and Braak. (1991) Neuropathological staging of Alzheimer-related changes.

However, the predictive validity of this concept is doubtful, based on numerous observations made in populations of aged individuals who, despite the absence of neurological signs, have brain synucleinopathy ranging up to Braak stages 4 to 6 at postmortem. Furthermore, there is no relation between Braak stage and the clinical severity of PD. Braak staging in AD has six stages — I through VI. The staging focuses on the location of NFTs. Stages I and II are when the NFTs are limited to the transentorhinal region of the brain. Stages III and IV are when the NFTs are in the limbic regions, which includes the hippocampus. The Braak hypothesis is offered to explain the onset of spontaneous Parkinson's disease (Staging of brain pathology related to sporadic Parkinson's disease.) According to the Braak hypothesis, some unidentified pathogen such as a virus or bacterium is inhaled and ingested.

Most importantly, this assessment of the Braak staging scheme suggests that there is insufficient evidence to claim that human PD begins in non-dopaminergic structures of the brainstem, and then evolves in a caudal-to-rostral progression over the course of the disease. A number of currently unresolved questions lead to this assessment.
